Hip pain when walking, at night, or spreading into the groin? Hip-area pain has many causes and it's far from always joint wear. The key is finding where the pain truly comes from.

Hip pain is tricky because a surprising number of sources project into the hip area: the hip joint itself, the muscles and tendons around it, referred pain from the lower back or the sacroiliac (SI) joint, and sometimes a cause outside the musculoskeletal system altogether. That's why, with the hip, finding the source comes first.

Where it hurts is only a clue, not a diagnosis

People describe pain by location — front/groin, side, or back near the buttock. That's a useful clue, but only a rough one. One source can show up in several places, and one place can have several causes: a tight hip flexor (iliopsoas) can hurt in the groin and also near the SI joint; a gluteal problem can project to the side and the groin; and low-back pain can masquerade as hip pain. So location can't be trusted as a diagnosis — an examination decides.

The key step: differential diagnosis

According to studies, hip pain in adults has many distinct causes and a correct diagnosis is the prerequisite for effective treatment. I distinguish mainly between a structural cause (e.g. osteoarthritis), a neurological or referred cause (from the lower back or SI joint), and a functional cause (muscles, tendons, coordination). Only then does it make sense to choose treatment.

Hip osteoarthritis doesn't automatically mean surgery

According to international guidelines, the foundation of hip osteoarthritis treatment is education and a structured exercise programme — exactly what physiotherapy does. Exercise can't rebuild cartilage, but it markedly reduces pain, improves mobility, and can delay or even avoid the need for surgery. Surgery comes when conservative care no longer suffices.

Please note: Groin and hip pain can also have causes outside the musculoskeletal system (a hernia, gynaecological or urological problems, swollen lymph nodes). Pain with swelling, fever, a lump, or urinary/digestive problems, or after a fall with inability to bear weight, should be assessed by a doctor first.
