An ankle sprain is the most common musculoskeletal injury. Most people treat it as a minor thing: a few days of limping, the swelling goes down, and that's it. But that's where the problem starts. An ankle that heals but isn't properly rehabilitated stays weak and unstable — a direct route to the next sprain and chronic instability.

Why a sprained ankle must not be underestimated

According to studies, a previous ankle sprain is the single strongest risk factor for another one. Around 20–40% of people develop chronic ankle instability after an acute sprain — the ankle repeatedly gives way and you stop trusting it, which over time raises the risk of early arthritis. Crucially, the earlier functional rehabilitation starts, the better: delaying it markedly increases residual instability. The later, the worse.

The acute phase: what to do right away

The modern approach is summed up as PEACE and LOVE, which replaced the old "just ice and rest": protect and offload briefly, elevate for swelling, avoid things that hamper healing in the very acute phase (be cautious even with anti-inflammatory drugs), and gradually load and move as pain allows — because movement supports healing more than long rest.

How I rehabilitate a sprained ankle

According to guidelines, an active exercise programme beats passive rest. The aim isn't to return the ankle to its pre-injury state, but to make it stronger and more confident than before: restore movement, build strength and endurance, retrain balance and reactions (this is where recurrence is decided), and return to sport by objective criteria, not by the calendar. A brace has its place as prevention when returning to sport, but it's not a substitute for rehabilitation.
