Jumper's knee and runner's knee are typical overload injuries in athletes and recreational exercisers. They're not caused by a single accident but by tissue that can't keep up with the load. The good news: most cases resolve with targeted exercise, without injections or surgery.
Where it hurts
- Jumper's knee (patellar tendinopathy): pain just below the kneecap. Typical for jumping sports, squats and lifting. Worse with jumps, squats and going down stairs.
- Runner's knee (IT band syndrome): pain on the outer side of the knee. Typical for runners, worse after a certain distance and running downhill.
Overload, not injury
Both develop gradually when tissue gets more load than it's currently prepared for — often after a sudden increase in training, insufficient strength, or technique that overloads the knee. Rest calms the pain but weakens the tissue, so it returns on the way back to sport. The way out is controlled, progressive loading.
How I treat it
According to studies, exercise therapy is the first-line treatment for lower-limb tendinopathies, with good long-term results. I find what overloads the knee, adjust the load, and build a targeted strengthening programme. Crucially, the knee rarely fails alone — with runner's knee the hip and glutes are often weak, with jumper's knee the landing technique. So I address the whole chain, not just the sore spot.
