Tennis elbow (outer side) and golfer's elbow (inner side) are two sides of the same coin — a tendinopathy, an overload problem of the forearm tendons where they attach at the elbow. You don't need to play tennis or golf; it most often affects people doing manual or repetitive work, or long hours at a computer.
Overload, not inflammation
These problems used to be called inflammation, but we now know it's mostly a change in tendon quality from repeated overload. That's why anti-inflammatory creams and rest alone don't fix it — the tendon needs controlled load to rebuild and get stronger.
How I treat it
1. Ruling out other causes
Not everyone arrives with a clear diagnosis. Outer-elbow pain can also be a nerve entrapment or referred from the neck, so first I confirm it's really a tendinopathy.
2. Calming and isometrics
Releasing the overloaded tissue, then isometric loading that eases pain and starts loading the tendon safely.
3. Eccentric strengthening
The core of treatment. According to studies, eccentric exercise is a first-line treatment for elbow tendinopathy, rebuilding the tendon so pain doesn't return.
4. When it's not enough: shoulder & posture
If the elbow alone isn't enough, I address the shoulder blade and whole arm so the elbow isn't repeatedly overloaded.
Braces and injections
An epicondylar brace can help when the muscle-tendon system is overloaded, but not in every case. Repeated corticosteroid injections are best avoided — they give quick relief but can harm tissue long term. The focus is on exercise.
